Learning Model Commitment Form

On March 1, 2021, the NLMUSD School Board voted to reopen elementary schools:
Preschool and Grades TK – 2 on March 29, 2021
Head Start and Grades 3 – 5 on April 12, 2021

Elementary school parents/guardians have the option to choose if their child remains in distance learning or returns to school for in-person instruction in a hybrid model.

Please select your option by completing our online form by Monday, March 15, 2021. The option you select for your child(ren) is for the remainder of the school year. If we do not receive your commitment form by March 15, 2021, your child will be placed in the revised distance learning model for the remainder of the year.
